This is a combined synopsis/solicitation for commercial items prepared in accordance with



the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with



additional information included in this notice. This RFQ will also use the procedures under



FAR Part 13. In accordance with 13.106-2, the contracting officer may take into



consideration information such as the contracting officer's knowledge of and previous



experience with the supply being acquired or other reasonable basis. This announcement



constitutes the only solicitation; proposals are being requested and a written solicitation



will not be issued.





The reference/solicitation number is VA-263-11-RQ-0901 and the solicitation is issued as a



request for quote (RFQ). The combined synopsis/solicitation will result in a contract and



corresponding purchase order.





The provisions and clauses incorporated into this solicitation document are those in effect



at the time of publication. Provisions and clauses incorporated by reference have the same



force and effect as if they were given in full text. The full text of the Federal

This requirement is for the purchase, delivery and installation of 157 patient beds for VA



Black Hills Health Care System locations in Hot Springs, Rapid City, Sturgis, and Pine Ridge



South Dakota.





This solicitation document and incorporated provisions and clauses are those in effect



through Federal Acquisition Circular (FAC) 2005-38. Only emailed requests received directly



from the requester are acceptable.





Delivery and acceptance of deliverables will be FOB destination within 45 days of receipt of



order to specified locations.





The provision at FAR 52.212-1 Instructions to Offerors - Commercial Items, applies to this



solicitation and incorporate the following addenda to 52.212-1, 52.204-6 Data Universal

award a firm fixed price contract resulting from this solicitation to the responsible



offeror whose offer conforming to the solicitation will be most advantageous to the



Government, price and other factors considered. The following factors shall be used to



evaluate offers: Technical Capability to meet the governments requirements.

GENERAL INFORMATION - STATEMENT OF WORK



INTRODUCTION: This requirement is for the delivery and installation of patient beds with



matching underbed three drawer storage units. Beds shall be Platform Twin Bed design with



Closed Sides, one side open for underbed drawer storage units. Bed must be brand name or



equal to "Kwalu model PHBET41". The bed should allow for a three drawer storage unit to fit



underneath. On the opposite side of the drawers the bed should be closed off. Underbed



Drawers must be brand name or equal to "Kwalu model PHDU3 Phoenix". Storage drawers shall



fit underneath the bed and be of a matching color. Beds and underbed drawer storage units



shall have scuff-, scratch- and water-resistant finish with antimicrobial properties for



aiding in infection control protocols. All screws and hardware shall be tamper resistant and



of high quality to endure heavy use.



SCOPE OF WORK:



Deliver and install beds to the domiciliary areas as follows:



Hot Springs – 139 Beds:



Bldg 8 - Wards A and B

Bldg 7 - Wards A and B

Bldg 6 - Wards A

Bldg 5 – Wards A and B



Rapid City – 6 Beds



Ft Meade – 5 Beds



Pine Ridge – 7 Beds



Work areas shall be left clean of dust and debris. All furniture packaging shall be removed



from VA property by the vendor at the end of each work day. Furniture packaging is to be



recycled by vendor if possible.



Vendor installation team shall be required to transport furniture from VA warehouse onsite



to domiciliary buildings onsite. Work shall be performed on weekdays during normal business



hours: 8:00 a.m. to 4:30 p.m.

STATEMENT OF WORK PART C – MISCELLANEOUS INFORMATION



Special Considerations: Vendor shall provide a work schedule for the completion of bed



installations at all sites. The work schedule shall be coordinated with the nursing staff at



each site in order to facilitate patient moves with as little disruption as possible.



Variances in the schedule must be approved by the nursing staff in advance. Music or other



unnecessary audible activity is not allowed in the domiciliary. Individuals performing the



installation must wear VA assigned contractor badges at all times, dress appropriately in



clothing that does not display references to any commercial product(s) other than the vendor



itself and use appropriate language at all times.



Contractor Furnished Materials: All materials and tools to complete the work identified.



Government Furnished Materials and Services: The Government will provide office or meeting



space to discuss projects or plans.



Qualifications of Key Personnel: Experienced Furniture Vendors and Installers Only.



Security Requirements: Vendor badges to be worn at all times.
