CONTINUATION OF SF-1449 - BLOCKS 17a - 25

(1) Block 17a: Contractor shall include company DUNS number.

CONTRACT ADMINISTRATION:


The U.S. Coast Guard Surface Logistics Center will perform all contract administration under this contract. The location of the Contracting Officer and Contract Specialist for the contract is at 300 East Main Street, Suite 600, Norfolk, VA 23510. Communication pertaining to the contractual administrative matters shall be addressed to the Contracting Officer and Contract Specialist listed below. Changes in or deviation from the scope of work will not be effected without a written change request and modification to the contract as executed by the Contracting Officer.


The Contracting Officer for this requirement is Ms. Shavonda Jamison. She may be reached at 757 441-7036 or Shavonda.Jamison@uscg.mil.


The Contract Specialist is Timothy Shuhart. He may be reached at 757 628-4602 or Timothy.M.Shuhart@uscg.mil. The Contract Specialist services as the point of contact between the Contractor and the Contracting Officer. This person does not have a warrant that provides the authority to contractually commit the government.


The Contracting Officers Representative (COR) is CWO Robert Cline. The COR may be reached at 757 620-9013 or Robert.W.Cline@uscg.mil. The COR is the government personnel designated in writing by the Contracting Officer to act as a technical advisor for the Contracting Officer for terms within the scope of the contract. A designation letter will set forth the authorities and limitation of the COR under this contract.


DESCRIPTION OF WORK:


(a) The Contractor shall furnish all necessary labor, material, services, equipment, supplies, power, accessories, consumables and such other things as are necessary, except as otherwise specified, to perform dock side repairs and alterations to the vessel, in accordance with this solicitation and attached specifications.


(b) Failure to read the specifications in their entirety will not relieve the offeror from the responsibility for properly estimating the difficulty or price of successfully performing the work. Failure to do so will be at the offeror's risk. All quotations submitted in response to this solicitation shall stand firm for 60 calendar days from the response date of this solicitation.


ALL CONTRACTORS ARE RESPONSIBLE FOR VERIFYING THE RECEIPT OF THEIR QUOTES.


REQUIRED DOCUMENT:


Each offeror shall furnish the information required by the solicitation, which includes:


1. Signed 1449 with offeror's address, DUNS and other information completed.
2. Completed USCGC BLOCK ISLAND SCHEDULE on Page 5
3. Past Performance Documentation as outlined on pages 30/31
4. Small Business Certification annotating as a HUBZone certified concern
5. Welding Certifications and Procedures as outlined on page 29
6. Statement of Insurance Coverage from current Insurance carrier as outlined on page 16

PLACE OF PERFORMANCE: The place of performance will be at USCGC BLOCK ISLAND (WPB-1344), located at 2301 East FT. Macon Road, Atlantic Beach, NC 28512.


ANTICIPATED AWARD DATE: The anticipated award date is on or about 27 December 2013, however it is subject to change. The government intends to award a fixed priced contract resulting from this solicitation to the responsible vendor whose complete quote, best and final offer, conforming to the solicitation, provides the best value.


PERIOD OF PERFORMANCE: Work covered by this solicitation shall be completed within thirty two (32) calendar days from the date the vessel becomes available to the Contractor. The availability is scheduled to start on or about 25 February, 2014 and end on or about 28 March, 2014. A planning document as specified in section 3.2.5 of the SFLC Standard Specification 0000 will be required seven (7) days prior to the arrival conference. An arrival conference normally will be held within 48 hours of the scheduled start date.


QUESTIONS AND REQUEST FOR DRAWINGS: The last day to request drawings or ask questions regarding this solicitation is 7 November, 2013 at 1:00PM EST. After this date further requests may not be accepted due to time constraints. CONTRACT TYPE: This solicitation will result in the award of a firm fixed price contract.


QUOTATION DUE DATE: Quotations must arrive no later than 1:00PM EST, 14 November, 2013. Quotations may be sent via electronic mail (e-mail) to Timothy.M.Shuhart@uscg.mil, or quotations may be sent via the US Postal Service or hand delivered to: USCG Surface Forces Logistics Center, Attn: Timothy Shuhart, 300 East Main Street, Suite 600, Norfolk, VA 23510. As per FAR 52.215-5 FACSIMILE QUOTATIONS (OCT 1997), vendors may submit facsimile quotes as responses to this solicitation. E-mail and Facsimile proposals are subject to the same rules as paper proposals. The telephone number for receiving facsimiles is 757 628-4628.


PLEASE NOTE: QUOTATIONS THAT DO NOT INCLUDE ALL REQUIRED DOCUMENTS MAY BE CONSIDERED INCOMPLETE AND MAY NOT BE CONSIDERED.


SCHEDULE OF SUPPLIES/SERVICES: Base Contract Line Item Numbers (CLINs) are marked as "D" for Definitive. Optional CLINS are marked "O" for Optional. Insert unit prices in all CLINs listed in the Schedule of Supplies and Service Attachment unless "NSP" is printed in the Unit Price column for any CLIN. "NSP" means "Not Separately Priced."


TRAVEL AND PER DIEM CLIN: CLIN D-00C "Travel and Per Diem" is applicable only to a contractor not within the local commuting area (50 Miles) of the vessel's home port. It is anticipated that any contractor within the 50 mile commuting area will enter zero (0) or "N/A" for the unit and extended price for this CLIN, on the schedule of supplies or services. The vessel will be located at Atlantic Beach, North Carolina.
